This policy describes what Peptides.Consulting ("we," "us") collects, how it is used, and the choices you have.
What we collect
If you complete the assessment, we collect the information you provide: your name, email address, state, your answers to the assessment questions, and your generated summary. If you request a consultation, we also collect your preferred time and any note you include. We additionally collect standard analytics data such as pages visited and the campaign that referred you.
How we use it
Assessment information is used to create your conversation summary, to respond to your requests, and, where you have asked for a consultation, to coordinate clinician review with our clinical partner. Analytics data is used to understand how the site is used and to improve it.
Who we share it with
Assessment and consultation information may be shared with our clinical and operational partners (including Lyv Health) for the purpose of provider review and scheduling. We use service providers for hosting, form processing, and analytics. We do not sell your personal information.
Retention and your choices
We retain lead information for as long as needed to respond to your request and meet legal obligations. You may request access to or deletion of your information at any time by contacting us at privacy@peptides.consulting.
Not a medical record
The assessment is educational. Information you submit through this website is not a medical record. If you proceed to care with a clinical provider, their own privacy practices and notices will apply.
